Junior Club Team Confirmations, Changes & Division Requests

Use this form to provide a complete list of ALL teams your club intends to enter for the upcoming season.

Please include any team name change requests and indicate your preferred division for each team where applicable.

All teams listed on this form must also be registered on Spawtz in the appropriate age group for the new season, and the relevant team entry fees must be paid online.

Please submit one form per club covering all teams by the published deadline.

If you need to amend your submission, please submit a new form containing all teams again, as only the most recent submission will be used.
